Office Policies 

At Endodontic Associates of Greater New York, PC we make every effort to provide you with the finest endodontic care. For your convenience we accept all major credit cards, personal checks, money orders and cash and payment is due at the time service is rendered. Professional fees vary according to the procedure and type of tooth treated. Please be aware that we do not accept any form of insurance as payment. We will, however, work hand in hand with you to maximize your insurance reimbursement for covered procedures. Please be aware that dental insurance is a contract between you, your employer and the insurance company. Questions concerning coverage, deductibles, co-payments or yearly maximums should be directed to your employer or insurance carrier.

Appointment

To schedule an appointment, please contact our office by phone at  (212) 355-4444 or complete an online appointment request.

For your convenience our office will call to confirm your appointment 48 hours before your scheduled visit. If you need to cancel your appointment with us, we would appreciate notification as early as possible. If you fail to cancel your appointment within 24 hours of your scheduled visit, a fee may be incurred.

Emergencies

Endodontic Associates of Greater New York, PC is open Monday-Friday from 8:30 am until 4 pm. We will schedule your appointment as soon as possible but same day appointments are usually available for emergency situations.  We generally allow time in our schedules to accommodate emergency situations, however, various circumstances may require patients to wait until one of the doctors is able to see them. In these situations we appreciate your understanding and patience. 

Patient Forms

If you are a new patient to our office, please visit our patient forms page to download our Medical History Form Page 1 and 2, the Pharmacy Information Form, and the Payment Arrangements Form.  Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival.  Please contact our office if you have any questions at all.

Payment Options

For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed.  Feel free to contact our office if extenuating circumstances require a discussion of your financial arrangements.

     
Dental Insurance

Our office does not accept insurance as payment for our services, however, we make every effort to maximize your covered reimbursement. Please contact our office for insurance details.  We look forward to hearing from you.
